Parent E-mail mailing list, and also to express your volunteering preferences.
E-mail
addresses and phone numbers tend to change rapidly, so please be sure to update your information at least annually.
Information gathered here will be used only for band booster related
communications and will not be shared with anyone outside band and the Salem
Band Boosters. Primary uses will be to notify parents of immediately
useful band booster information, such as volunteering needs, fundraising
information, trips to band contests, etc.
Our form requests only the information we think we will need (for e-mail,
volunteering, and fundraising contacts). However, if you only want to
provide some of the information listed, that's fine with us too. If you've
already provided this information in paper format, you don't need to duplicate
it here (but you are welcome to do so!). Please press the submit button
at the bottom of the form when you are done!
